Change of Legal Name

In order to amend the name on your Licence, you will need to provide us with a copy of your passport in your new legal name. Once you have submitted the Change of Details Form below please email a copy of your passport and evidence of the name change such as a marriage certiificate, decree absolute, change of name deed to licensing@clc-uk.org.

As per our Universal Guidance the CLC will only issue a licence in an individual’s legal name as stated on their Passport, therefore should you wish for the name on your licence be amended, this would need to reflect on your passport.

Please also note that amendments to a licence incur a fee.
